COBRA coverage

The Consolidated Omnibus Budget Reconciliation Act (COBRA) is a federal law enacted in 1985. It requires most insured and self-insured group health plans to provide continuation of coverage to employees, spouses and dependents who lose coverage for certain reasons.

Out-of-area coverage

Traveling or have dependents who are out of the area? Your large group plan might provide coverage. Check your member certificate to see if you have coverage.

Prior authorization

Some provider visits or medical services require prior authorization by Prevea360 Health Plan. This helps us manage your employees’ care and ensure they get what they need and if it’s medically necessary.

Your employees should ask their primary care provider about the need for prior authorization before receiving the service or going to a specialist.
