How can I get a reimbursement form for someone who does not have access to a PC?

Contact the Customer Care Center at 877.230.7555. We will be happy to mail the reimbursement form to you. It should arrive in three to five business days. Please complete the form, attach your receipt and return it to Prevea360 Health Plan.

Do I need a prescription from my doctor to get this shot?

No. The routine flu shot is a covered benefit, and you do not need a prescription to receive it.

How long will it take to receive reimbursement?

Prevea360 Health Plan will process the claim within 45 days after receiving the receipt and claim form.

Is there a certain time frame in which I can get the shot?

You may only receive one reimbursement per calendar year.

What if my flu shot was more than $27?

You will be responsible for any remaining balance.

Can I be reimbursed for immunizations paid for with a flu shot gift card?

No. Prevea360 Health Plan cannot reimburse flu shots paid for with flu shot gift cards.

If I get the flu shot at a Prevea Health clinic or from a Prevea360 Health Plan provider, will I still need to pay out-of-pocket first and be reimbursed later?

When you see a Prevea Health or Prevea360 Health Plan provider and receive your flu shot through a provider's office or clinic, the claim will be submitted by your physician and covered at 100 percent through your policy with Prevea360 Health Plan.

What are my options if I lose or can't find my receipt?

You can ask for a duplicate receipt from the vaccine administrator. Prevea360 Health Plan will not reimburse you if you do not have a receipt for the flu vaccine.

When you say the shot can be received anywhere, does this include at work and with non-plan providers?

Yes. You can receive your flu vaccine anywhere, including non-plan providers, and receive the $27 reimbursement. If you receive the vaccine from a non-plan provider, simply pay for the service at the time you receive it and send a copy of your receipt with the reimbursement form.

Will you pay for the flu mist?

Following the Center for Disease Control’s (CDC) recommendations, please note that during the 2016-17 flu season, the FluMist vaccine, which is administered through a nasal spray, will NOT be a covered benefit for Prevea360 Health Plan members.

The injectable flu vaccine will be a covered benefit and available at our plan provider locations that offer the flu vaccine.
