Individual + family plans
We're here to help make Prevea360 Health Plan coverage as clear and understandable as possible because we know insurance can be confusing. The important part is that you continue to get the coverage and care you need.
Marketplace plans
Plan resources
Also referred to as the Exchange, Marketplace is a website (healthcare.gov) where individuals can compare plans and purchase health insurance. Marketplace plans are great for individuals and families that are eligible for discounts or financial subsidies. These discounts may include:
Tax credits
Tax credits lower the monthly premium you pay for health coverage and are more available than ever due to the 2021 American Rescue Plan act. Depending on your household income level, these credits can pay for a considerable amount of your premium. You can use your tax credit on most Marketplace plans.
Cost sharing reductions
- Lower the amount you pay on health care services.
- Cost-sharing reduction is only available on silver plans. There are three levels of cost sharing reduction based on income brackets:
- 100–150 percent of FPL
- 151–200 percent of FPL
- 201–250 percent of FPL
Here are some guidelines for eligibility:
- Individuals making $20,120 - $58,320 or more**
- Family of two making $27,213 - $78,880 or more**
- Family of three making $34,306 - $99,440 or more**
- Family of four making $41,400 - $120,000 or more**
** All income ranges and financial help (advanced premium tax credit) qualifications are determined by Healthcare.gov. Visit HealthCare.gov for more information.

Metal tiers
All Individual and family plans are divided into metal categories. These categories show how you and your plan share costs. No matter which category your plan falls in, the quality of care does not change.
Bronze plans
Bronze plans are best if you do not typically need to see your doctor or use your prescription coverage very often. The monthly premium payment will be lower, but when you do need care, your out-of-pocket costs will be higher.
Silver plans
Silver plans have your out-of-pocket costs and your monthly premiums more evenly balanced. These plans are beneficial if you may use your health care for more than just preventive care.
Gold plans
Gold plans are ideal and cost-efficient if you anticipate going to your provider or pharmacy frequently. These plans will have a higher monthly premium payment, but your out-of-pocket costs will be lower.
Special enrollment information
Outside open enrollment, you can enroll in new coverage or make changes to your existing policy if you have a certain life event that qualifies you for a special enrollment period.
